Chicago Craniofacial Pain & Rehabilitation office is located at 401 W. Prospect Ave, Mt. Prospect IL. It is in the Smiles By Smoron building. Both office share the reception space and front desk staff. So don't be alarmed if you call the office and get a Smiles assistant, both office teams are trained to help schedule your appointment and direct your questions to the appropriate staff member.
Payment & Insurance
We are currently an In-Network BCBSIL PPO Medical and Aetna Medical Insurance provider and will verify your co-pays and deductibles for your visit. Visit cost will vary based on your individual plan coverage.
We accept HSA and FSA payments and will provide super bills for submission to any Out of Network insurance.
We are a billing free office. Visit co-pays, deductibles and fees are due at the time of service.
